Consent Forms

You will need to fill out and return a consent form for your child to attend a Peak Pursuits Summer Activity Week.  This form will tell us any medical information and requirements for your child and give us emergency contact numbers should we need to contact yourselves.

Who will be looking after my child?

During your child’s time with us they will be looked after by Peak Pursuits Team Leader.   Each Team Leader will act as a friend, mentor and role model to your child, caring for their well being and ensuring they have safe fun packed week.

Who are the Peak Pursuits Team Leaders?

Peak Pursuit Team Leaders are enthusiastic about the outdoors and enjoy working with young people.  They have all had previous experience in working with all kinds of young people and are sensitive to young people’s issues.   All Team Leaders are subject to a CRB check and hold a first aid certificate.  We select out Team Leaders for their qualifications, experience and enthusiasm.  All our team strive to give your child an amazing experience they will never forget.

What health and safety procedures are in place?

The safety of young people is paramount at Peak Pursuits.  All activities and locations have being risked assessed and an ongoing risk assessment is also in place.   Peak Pursuits staff development program ensures all Team Leaders are up to date with current health and safety procedures and their skills are constantly assessed and developed.  Peak Pursuits holds an AALA licence ensuring that we meet the high standards required to take young people into the outdoors for adventurous activities.  We are an approved provider for the Local Authorities and work with many schools and youth originations such as the Scouts and Girl guides.  We pride ourselves on young people experiencing and learning in the outdoors in a safe and fun environment.

What will happen if my child has an accident?

If your child has an accident and will require medical treatment we will contact you immediately.  It is important to ensure you have filled out the consent form clearly and correctly and it is given back to us before the week begins.  If in the unlikely event we cannot contact you, Peak Pursuits Team Leaders who are all first aid trained will act in best interests for your child and make any decisions that are necessary to help your child.

What about the activities?

Your child will be in a group for all their activities.  We have chosen a range of activities for your child to get maximum enjoyment out of the week.  Canoeing and climbing will be at locations in North Staffordshire in the picturesque Peak District, mountain biking will be at either Cannock Chase or Delamere Forest.  The music, art and dance activities will be held Peak Pursuits HQ in the village of Audley, Stoke-on-Trent.  All Peak Pursuits Team Leaders hold the relevant National Governing Body Awards to run such activities.

What if my child regularly takes medication?

For your child’s welfare it is important Peak Pursuits Team Leaders know your child’s medical history. You will have filled out any relevant medical information in the consent form, ensure this is clear and correct.  Your child will need to bring their medication if required to take during the day, this includes inhalers even if they hardly or never need to use it.   All medication must be clearly labelled with your child’s name, dosage and time or occasion to be taken and handed in to a Peak Pursuits Team Leader in the morning.

Who will my child be with?

Your child will be with other likeminded children during the week.  If your child is worried they won’t know anyone, don’t worry as a Peak Pursuits Activity Week is a great place to make new friend.  Our Team Leaders will ensure they are looked after and make new friends.  If your child wants to bring their friend along; that’s great – the more there merrier! They will need to go through the same booking procedure as your child and ensure there are enough place for that week.

How will my child benefit coming to Peak Pursuits Activity week?

Your child will face new challenges, meet new people and enjoy new activities; it will be an adventure climbing up rock faces, navigating their way through the woods on a mountain bike, learning to play African drums.  There will come away with memories and tales that will astound the bravest adventurer or Parent!

How will my child get to the activities if they are not at Peak Pursuits HQ?

For activities away from the Peak Pursuits HQ, your child will be transported in a Peak Pursuits Vehicle.  All staff undergo driver training with Peak Pursuits Transport Co-ordinator and hold a D1 on their licence if driving mini-buses.  Whilst your child is travelling in a Peak Pursuits vehicle they will be required to wear a seatbelt and no young people are permitted to sit in the front of our vehicles.

What will happen at the beginning and end of the day?

You will be able to drop your child off with us at Peak Pursuits HQ in Audley, Stoke-on-Trent between 8:00am and 9:00am.  There will be a Team Leader to greet you and your child, it is important you child signs in with us so we have them accounted for.  We will provide Juice and toast with in this time, but if your child arrived after 8:45pm we may not be able to offer this service.  There will be a games console for your child to play on with other young people until we are ready to start the day’s activity.
